RankOne Instructions

To participate in a sport at Central York Middle/High School the following requirements must be met prior to eligibility:

  1. Physical: A sports physical must be completed by a Licensed Physician, PAC, or CRNP on PIAA paperwork (Section 6 ONLY).
    • Physicals must be completed NO EARLIER THAN June 1 of the current school year to be valid for the upcoming academic year.
    • Physicals are valid for the ENTIRE academic year.
    • The most recent Section 6 can be found in your Rank One account. Rank One directions below.

PLEASE DO NOT EMAIL THE COMPLETED PHYSICAL, THIS IS A HIPAA VIOLATION, EMAILS WILL BE DELETED.

  1. Rank One Account: Every student-athlete must have an online Rank One Account. This account will need to be updated each year and should be created using a valid PARENT email address.

New Rank One Users:

  1. Go to www.rankone.com and click on Create a New Account. Enter your (parent’s) first name, last name, and email address. You will then receive an email asking you to set up your password. Passwords must be at least 6 characters long. Once you have your password and sign in, you can enter your student’s ID number. Student ID # is found in Skyward – click on Student Info the # is to the right when you click Student Info.
  2. You may link multiple students under your parent account. Once you set up your account click the Start Forms link. This will bring you to the home page.
  3. Click on the View link to see the forms that need to be completed.
    • Follow the directions under each link. (In the Fall season there are 5 documents, Winter and Spring there are 6).
    • If you compete in a Fall sport, you will only need to complete Section 7 (Recertification Form) in the Winter and/or Spring.
    • Complete Sec 5 and 6 then PRINT BOTH FORMS – take them to your physical.

TAKE SEC 5 & 6 TO YOUR PHYSICAL APPOINTMENT

Returning Rank One Users:

  1. Login to your account, complete the PIAA Forms (Sec 1-6) and other forms indicated.
  2. If you forgot your password, click on the click here link next to forgot password. If you are not sure if you have an account, you can search by clicking on forgot your account button.

Physical verification:

  1. Once the physical is verified you will see a GREEN check mark to the right of your child’s name.
  2. A RED check mark means they are not cleared. Please download the app to receive notifications.
  3. To see which documents have been completed go to FORMS, those completed will have a YES and completion date. Those not completed will have a NO.

Physicians must complete the following on Sec 6 to be accepted:

  1. Completed: MEDICAL & MUSCULOSKELETAL Sections.
  2. Marked: CLEARED, NOT CLEARED, CLEARED WITH RECOMMENDATIONS (Dr’s recommendation must be completed prior to clearance)
  3. Bottom 3 lines completed in their entirety.
  4. Dr’s address stamp anywhere on the bottom of Sec 6
  5. Once completed, upload Sec 6 ONLY into Rank One
